Documents you need to send
Complete form AP1 to change your name on the title register. Read the guidance to get help completing form AP1. There is also a "Youtube" video which may assist you.
Send the completed form with:
- a certified copy of the deed poll, statement of truth or statutory declaration
- a copy of an official document in your former name, for example, a driver's licence passport, or utility bill
Evidence of Identity is usually required for the person changing their name unless they are represented by a conveyancer. See Practice guide 67: evidence of identity or more information and the options available to you.
There is no fee for this type of application
