Completing a DS1 Form

Completing the DS1 form as solicitor who logged the charge is refusing to remove it even though the charge has been satisfied . Can someone please advise what i enter in Box 5 for Lender and Box 8 for Execution ?

Debby - the lender needs to complete the form DS1 
The side notes on the form explain what's required and the lender will need to execute the form. If you do then it will be rejected.
If you have paid off the legal charge and the lender is refusing to discharge it I would recommend seeking legal advice. You are likely to need to take legal action against the lender to secure the form DS1 properly executed or ask the judge to do that if the lender still refuses
