Transferring part of land and adding to existing title

I am completing a TP1 and AP1 to transfer a small area of land.  The transferee wants to add this to an existing title, rather than this remain on a separate title.  Is this possible to do within these forms?

J D - The request to amalgamate the transferred land with the title involved must stated in Panel 4 of the form AP1.  The title number to which the land is requested to be added should also be listed as part of the application.
