Transfer my share of house to ex

I have separated from my husband and need to transfer my share of our house to his name. There is no mortgage, he has already given me £75000 for my share. Which forms do I need to fill in and what fees category would apply? 

A Fernley - You both need to transfer the whole of the title to his name.
