Completing an AS1 Assent form by typing

Hello,
I have lost the use of both index finger and thumb in both hands and I can no longer write efficiently. I am able to sign my signature and was wondering can I type in the boxes and sign by ink where required on the ASSENT form?

Hello Simon - yes you can. You can complete the WORD form on your device and then print and sign as appropriate
