AP1 form transfering land into a Limited company

I am transfering a small piece of land which is owned by( just) myself into my limited company which I am joint director with my husband. We are not using a  conveyancer. On the AP1 form section 14. I list my name as the Transferor but do I then add both myself and my husbands names as Transferees ( on behalf of the company ?) Likewise do we BOTH sign in section 15 ? 
In section 9 . The company address is the same as my personal adress. This is the address which is currently held by the land registry. Do mark the third box down ? Or the 4th and re-write the address? 
Section 4. What do I write in Applications in priority order ? Do I simply write Transfer ? Thank you

Nicky - the Company is the Transferee it seems so that name should be included in panel 14
Panel 15 should be signed by whoever is completing the form and submitting the application. If that's the Company then you can both sign as Directors. If it's the two of you or just one then sign as appropriate
Panel 9 should be completed as appropriate depending on which address(es) you want included for the Company. If that's complex then use the third option and complete as appropriate
Panel 4 would be Transfer, yes
