Re COG1, section 3.
You ask for copies of council tax and utility bills received by post. However I don't get bills by post from most of the firms I deal with and often don't keep the few I do get. And I don't have a mortgage.

Will you accept a bill from the company that maintains my alarm? 
Will you accept a bank statement?
Will you accept a copy of my driving licence - which shows my address?

David - panel 3 is an either/or scenario. List A includes a copy of your driving licence 
List B is an either/or so as you've satisfied List A you don't need List B
