Remove charge and restriction

I am part of a committee that look after a local Community Centre, back in 2012 they were given some money to be used for a particular purpose, I understand a charge and restriction were added in case this money was not used as planned.  No current members knew this charge was in place
That company is now being taken over and we have received a letter giving us permission to get the charge removed, they have then sent a DS1 form and told us to fill out a AP1 form and submit them together.  I think this is incorrect hence why checking.  I think the restriction is date related to the charge so we do not need a separate form for that but we need them to submit DS1, DS2 and ID2.  Please could you advise, the title if needed is ST303549.  They have said they cannot help after March so we need to action asap.

Helen - you can use either a DS2 or AP1 to submit so that's a moot point
The restriction is 'separate' to the charge as you state so we'd need an RX4 from the company or an RX3 from you - RX4 from the company would be the 'best' option
Identity verification for the company would be by way of a form ID2. Again 'best' if they submit the application as if you do we need you to have your identity verified as well
