Death of a title holder, removal of restriction and removal of a title holder from same title

I wish to 1)remove my deceased fathers name from the title deeds, 2)remove a restriction of a deed of trust and 3)remove my mother(to gift her share) from the title deeds to become the sole owner.
Can I do this all in one application or does it  have to be done in order
If so what order and what forms
