I am applying to remove a charge for a debt that's been repaid to me.Q.4 asks for 'Documents lodged with this form'. What might these be? The note in the margin talks about 'certified copies of deeds or documents' but unless I've been misinformed or misunderstood something I'll only be enclosing DS1 and ID3 and I can't see what else I would have to enclose.

Stewart - you'd list form DS1 and ID1 as documents lodged with the form DS2

Thanks for that, just one more thing: in Q6 after email address it says 'Reference'. Can i just leave this blank or do I need to put something?
