Hc1 form

Hello, I'm am filling out the HC1 form for historical editions of the title register. I'm not sure what date I need to write down, I need the edition previous to the current one now, as I need to show my mum and dad were joint tenants but since my mum passed the title has been updated. The title number is HD217886. Also will an official copy been sent ?

Hello - the previous edition date is 28th April 2014
Form HC1 is used to obtain an historical official copy 

Perfect great so is that the date I write on the form, also is the fee £11 ?
