Hello Sophie - if you are 'simply' reverting back to your maiden name following a divorce then ID1 is not generally required. It is required though if you have for example changed your name by deed poll, which some people may elect to do for own/other reasons.
The following explains what we require from you
Complete form AP1 to change your name on the proprietorship register. Read the guidance to get help completing form AP1. There is also a "Youtube" video which may assist you.
Panel 4 can be completed to refer to say "Change of name on divorce"
If you want to go back to your previous name, you can send a certified copy of either:
- the marriage certificate
- your final order (called decree absolute (for marriages) or dissolution (for civil partnerships) before 6 April 2022), if the process took place in your original name, not the married name
Or you can send a certificate from a solicitor to state that:
- The person known as (name on the register) is now known as (new name)
