What form do I need to change the legal ownership of out property from Joint Tenancy to benfic

My health is deteriorating and my wife and I want to place our shared home into a 50/50 beneficiary in kind arrangement. So if I enter nursing home her half of the house cannot be swallowed up by my fees.

we currently own it as a joint tenancy but would like to make us both into a simple 50/50 ownership model
