AP1 Form - Change of Name - Panel 14

Hi all, I am in the process of filling out the AP1 form to only change my name on the registry, and need help with panel 14 part 1.

As I am only changing my name, do I leave this section blank as there is no transaction taking place?

Hi Joe - that all depends on how the name change happened as with some you do need to have your identity verified

Has been changed by deed poll. Read that I'll also need to sort an ID1 form also.
