Removing rent charge using AP1

I have confirmation from the rent charge owner that the charge has been extinguished and I want to remove it from my registered title. I have been asked to fill in form AP1. The rent charge was registered with its own title.<br> <br> Do I need to list either or both myself and the company registered as the rent charge owner in panel 14(1), and if so as what?<br> <br> If so do I need to obtain proof of identity for the company (it appears to be a major property management company)? The charge was extinguished many years ago but not removed from the register.&#160;No solicitors or conveyancers were involved.&#160;
