Do I need to fill out seperate applications to update my name on two different title deeds?

I recently changed my name via deed poll so currently in the process of filling out an ID3 form to update title deeds.

However, do I need to fill out seperate applications to update my name on different title deeds? Or can I list multiple property addresses on one form?
