AP1 - Deed Poll

Hi, I am looking for support on the AP1 form. 
I know for name change via marriage certificate it is a little easier and simple as there are guides for that so my missus will be fine updating her name on the land registry for house jointly own however I have no idea what parts I should fill in and how for the AP1 form. 

I changed my name via Deed Poll. I require the fill in the AP1 and I will not be using a solictor as they advised that the forms are simple. 

Can anyone support me please?

Hi Adam - complete panels 1-7 inc and 15. If the name change is by Deed Poll then identity verification is also required - complete panels 12 and 14 and get her identity verified as appropriate

What do you mean by inc after the 1-7?

Adam - that means panels 1-7 inclusive so complete all 7 of the first panels
