Completing OC2

I am completing OC2 to see the property transfer information for 6 Avonhead Close Horwich BL6 5QD . Myself and my hisband Colin Whiittaker became owners of the property 15th July 2004 Title number: GM577939 and I want to view the property transfer form which which will hopefully show how we owned it eg joint tenany or tennants in common. However Im not certain which date to put on the form in pancel 7 when it requests date of document.
