Adding partner's name to the title deed

Dear. Sir

How can someone add the name of his wife on the title deed to show that they each hold the property 50% each of the property . There is no mortgage on the property 

Can they file the relevant form online using e-business account . They prefer not to use a Solicitor

I am remortgaging my property and I need to add my name to the title / deed as we have the new mortgage in our joint names. How can I do this? Thanks
