Double barreling name after marriage

I've recently got married but I want to double barrel my name only.
I've downloaded the AP1 form but there is no box to enter new name.
Could someone please advise me how I do this?

Lisa - you complete panels 1-7 inc and 15 of the form AP1. The 'new' name would go in panel 6 as 'the applicant' and make it clear what's required for example 'Lisa Baskerville-Davies - using double barrel name on marriage' or similar
