Declaration of trust

We helped our son buy his property by a gift of £39k some years ago, taking out a declaration of trust which was registered as a restriction on the title absolute.

We want to remove the restriction and cancel the declaration of trust so he owns the house as sole owner.  Can I do this myself and what form would I need to complete?
