Joint tenants or tenants in common

I am trying find out if a restriction form  A was on the title registry for a property I owned with my husband. We owned it together 15th july 2004 until his death in june 2009 -  Avonhead Close, Horwich, Bolton, BL6 5QD (Freehold)
Title number: GM57793

I am not certain what to request - do I fill in HC1 and if so which dates do I put on forvrequest of title register or do I need OC2 and request prooerty transfer form and again what date do I enter for the document in panel 7

Donna - The title number you have quoted appears to be for a property elsewhere.  I am assuming you meant GM577939. 

If you wish to establish if there was a form A restriction on the title register then you can obtain a copy of the historical register by completing the form HC1 and quoting any date from 29 July 2004 to 16 December 2008.  The register was amended on 9 July 2009 to remove your husband's name. 

You can also apply for copy of the transfer dated 15 July 2004 using the form OC2.  However, it would be useful to clarify that the transfer will not make any reference to 'form A restriction'.  The transfer will indicate if the proprietors wish to hold the property as joint tenants or tenants in common.  The form A restriction is only entered if there is indication of the property being held as tenants in common or if no information is provided.
