Land Charges AC6

I am about to send an AC6 by post.  The form says that we can request that the results are telephoned.  Will that be followed by the results in printed form, and we will need that.  Do I need to tick both the posted and telephoned request boxes at the top of the form?  

Irene - If you need the result to be telephoned and in a printed form then you should tick both the boxes.
