Lease Extension

I am in the process of extending my lease, the Freeholder/Landlord has completed ID1 form, however my solicitor is requesting the Management Company to complete ID2 form. My question is does the Management Company have to complete this or could it be myself, the Tenant/owner to complete ID1? The negotiation of extension was between myself and Freeholder/Landlord.
