Submitting a SEV form

Hello, I believe this is how I can submit a SEV form to Land Registry so can you confirm if this is correct and the document attached is sufficient?

Hello - the title number should have the letter prefixes before the numbers. And you need to complete panel 6 with your names and address It’s then a postal application
