Evidence of Identity for FR1 form

We are completing form FR1 and have filled in Panel 17 as per guidance.
I would like to know what evidence you require and when,

Mary - panel 17 of form FR1 is where the person applying adds their signature and date
Please refer to the explanatory side notes for guidance on completing the form
