Forms needed to remove ex-husband from title deeds

My ex husband and I divorced in January and the financial agreement was that the marital home be put in my name once the mortgage was paid off. The mortgage on the property was paid off in November.
What forms do I need to fill in and what fee is due to do this?
My ex-husband is still in full agreement to have his name removed so I was hoping it could be straightforward without the need to pay £500 to a solicitor (as I am on disability benefits)
Do I need form TR1 ?
