Form AP1 - deed poll name change

I want only to change my name on the register.  I have changed my name by deed poll and am having trouble with panel 14 (parts 1 and 2) on form AP1.

Panel 14 part 1
Do I fill this in or leave it totally blank?  I ask this as I am neither a transferor, landlord, transferee, tenant, borrower nor lender.  I am the property owner, who just wants to change my name on the register.

Panel 14 part 2
If I do leave part 1 blank, which box would I check here? Would I check the first box (ie. Evidence of identity "for each applicant in panel 6 is enclosed") ?

Tessa - you don't have to complete panel 14 but you do need to have your identity verified

To clarify - I don't write anything in panel 14 at all, neither in part 1 nor part 2 of panel 14?  

And would I indicate that I've had my identity verified by listing form ID1 (or ID3) in panel 5 of form AP1 (and of course enclosing the ID form)?

Panel 12: Confirmation of Identity
As I am registering only a name change, and not registering a transfer, do I leave this blank?

Panel 5: Documents lodged with this form
I understand that I need to list form AP1 (or AP3) and Deed Poll (certified copy). 
Should I also list my supporting identity documents as follows:
- Copy of personal details page of my passport
- Copy of personal details page of the passport of the person verifying my identity
- Copy of an official document in my former name

Tessa - correct re panels 12-14, blank
Panel 5 does not need to list the form AP1 as that's the application form that includes the panel 5
You need to provide a copy of the deed poll as you state but you also need to have your identity verified using form ID1/conveyancer or ID3/others as listed. The chosen form should then be listed in panel 5

In Panel 5 of form AP1, should I also list the passport copies (mine and my verifier's) and the copy of the official document in my former name, or not?

Tessa - you don't list anything that's part of the actual ID form. For example you would simply list form ID1.

I am looking at changing my name on the land registry for a house I jointly own. 
Are you able to provide any support on what you filled in on the full form and what you used for verfiying your identity?
It would be much appreciated.
