How to fill in RX4

I am not sure how to fill in box12. What should I write for "interest". Please tell me.

Masae - see the side notes re what's required here. If you are the named restrictioner then you don't need to state your 'interest'
However if you are not a named restrictioner, or their isn't one, then you need to state the basis on which you are the one signing the withdrawal
We can't tell you what to put as only you know the basis of you seeking to withdraw the restriction and the basis of why you are able to do that.
NB - if you are not the restrictioner or the person with the benefit of the restriction then it would be form RX3 and a request to cancel instead
