Adverse Possession Claim

Hello I’m about to submit an adverse possession claim for some land which has been used by us and previous owners of our house for over 20 years and over 15 years with exclusive gated access. 
I have completed the AV1 form and St1 and gathered stat decs from previous owners and statements from my current neighbours. I have also got satellite image of the land and a drawn map with further detail. 
My questions are: 

1. What Land registry address do I use to send the documents to? I live in Durham. 
2. do I need to include copies of title deeds or are they not needed  at this stage?
