Tenant amendment to a 10 year lease

Dear Land Registry

I have a ten year lease issued to the local doctors surgery that has had two partners leave and the remaining partners have requested that the lease be amended to reflect the changes.  Can you please advise on the process.

Nick - I assume the lease is registered under it's own leasehold title and in the names of the individual partners. If so then they can transfer the whole of the leasehold title as appropriate - Registering land or property with HM Land Registry: Change the registered owner name - GOV.UK
If that's not the case then can you share some specific details
