Adresses Incorrect on Land Registry Plan

I built a new house in the garden on an existing house in 2018. the original house number was 95 and had been 95 for over 100 years. The new house in 95A.
On the most recently land registry plan the numbers are incorrect, the old house is shown as 95A and the new house as 95. we are currently in the process of doing a split on these properties.
How do I go about rectify this issue, can my solicitor simply point this out to the Land Registry as part of the application and be sure it will be fixed?
And help or advice would be appreciated.

David - if you are applying to split the properties, and we agree to do so, then the new title plans should confirm the current details. Your solicitor should flag the new details also with the application
