Incorrect address for owners on Title

Hello, 

We've recently had our title updated after it taking two years to process and have noticed that on the title absolute section, although our names are correct as owners the address we reside at is wrong as it refers to the leasehold property.

How would we go about fixing this? We are unsure if it was an error on the original transfer form or from the Land Registry.
