RX1 for Form LL Restriction

I want to file a Form LL Restriction.  Do I need to apply for a Form LL?  Or do I register the restriction on form RX1?

It says on form RX1 for question 6 "Documents lodged with this form" - what doecuments to I need to provide if I am registering this restriction for the first time and where can I obtain them?

Quentin - form RX1 can be used to apply for a form LL restriction. There is no application form LL as it is a form of restriction only.
You only complete panel 6 if lodging additional documents but that wouldn't be necessary in this case
